eVNG USB Video Nystagmography (VNG) System: Dizziness and Vertigo Diagnostic System From BioMed Jena

The eVNG USB is BioMed Jena’s modular video nystagmography (VNG) system for assessing dizziness, vertigo, and balance disorders. BioMed Jena is a German company and one of the world’s five largest makers of audiology and vestibular diagnostic equipment. The eVNG records and analyzes the involuntary eye movements (nystagmus) linked to the function of the vestibular system in the inner ear, a method widely used to help assess the causes of vertigo and balance disorders such as benign paroxysmal positional vertigo (BPPV), vestibular neuritis, and Ménière’s disease.

The eVNG uses an improved image-processing algorithm that can extract the pupil from the video stream even if the patient wears eye makeup. It connects to a PC or notebook through USB 2.0, and no other computer hardware is required.

iHear is the exclusive authorized distributor of BioMed Jena in the UAE. It offers the eVNG to hospitals, ENT centers, and neurology clinics that diagnose dizziness, vertigo, and balance disorders, with local technical support and training.

What Is Video Nystagmography (VNG)?

Video nystagmography (VNG) works on a direct scientific principle. When the vestibular system of the inner ear is not working normally, the eyes can move involuntarily (nystagmus) because of the direct nerve connection between the inner ear and the eye muscles, known as the vestibulo-ocular reflex (VOR). The eVNG records these movements with a binocular video goggle that uses infrared illumination, and its software analyzes them automatically to show the type, strength, and direction of the nystagmus. This is information that helps the clinician tell a peripheral (inner-ear) cause from a central (brain) cause.

In short, the eVNG is a video nystagmography system that connects to the computer through USB 2.0. Its binocular video goggle records the movement of each eye separately at 100 frames per second (binocular 100 Hz, with a resolution of less than 0.2°), using a 1280 x 1024 pixel camera and infrared light. An improved image-processing algorithm can extract the pupil from the video stream even if the patient wears eye makeup.

The system is built on a patient database that can be accessed through the facility’s local network, and it is operated from one application that shows all the tests with the date of examination and the kind of test.

eVNG USB Clinical Uses: What Does It Help Assess?

The basic version of the eVNG includes spontaneous nystagmus testing, the first step in any vestibular assessment. It records any abnormal involuntary eye movement without an external stimulus, which may point to a vestibular disorder.

The caloric test is one of the most important tests of vestibular function. Each ear is stimulated separately with a stream of cool and warm air or water, and the eye response is measured. This can show a weakness on one side (unilateral weakness) or a directional preponderance, which helps the clinician work out where the problem is. The eVNG can be connected to BioMed Jena's KALORIstar Arctic, an air caloric irrigator with active cooling, so the whole test can be controlled with one button from the eVNG software.

The basic version also includes position and positioning tests, which are important in assessing benign paroxysmal positional vertigo (BPPV). BPPV is one of the most common causes of vertigo.

The system can be extended at any time with optokinetic tests, smooth pursuit tests, and saccadic and antisaccadic tests. These tests look at the brain's control of eye movement, not only at the inner ear. For rotational testing, the eVNG can be connected to the VESTAR 100 USB rotary chair.

An optional feature. To avoid the cost of an electrically driven rotary chair, the sinusoidal pendulum test can be done manually with an ordinary office chair, using the precise position sensors in the video goggle. The test is used to check how well a patient has compensated after an acute loss of vestibular function.

On request, the eHIT and eVNG can be combined in one system, giving a broader vestibular assessment that covers both the slow response (VNG) and the fast, high-frequency response of the vestibular system (the video head impulse test, vHIT).
EVNG USB Clinical Uses

Why Do Vestibular Specialists Use the eVNG USB Video Nystagmography System?

The eVNG USB is a modern video nystagmography (VNG) system used to assess the function of the vestibular system in the inner ear by recording and analyzing involuntary eye movements (nystagmus). It is used to:

Main areas of use:

Help assess peripheral vertigo caused by an inner-ear problem (such as vestibular neuritis and Ménière's disease).

Help tell central vertigo (linked to the brain or nervous system) apart from peripheral vertigo.

May be used to assess patients after head trauma and concussion.

Follow-up of patients with chronic balance disorders.

How Does the eVNG USB Video Nystagmography System Work?

The eVNG USB connects to the computer through a USB 2.0 port. Its binocular video goggle, with infrared illumination, captures the image of the eyes in real time, and the software then automatically does the following:

1

Detects nystagmus automatically and calculates all the relevant parameters.

2

Rejects artefacts automatically, to reduce errors in the readings.

3

Runs full test sequences automatically (spontaneous nystagmus, caloric test, and positioning tests).

4

Saves each patient's results in a patient database that can be accessed over the facility's local network.

eVNG USB FAQ: Common Questions About the Video Nystagmography System From BioMed Jena

The eVNG USB is a video nystagmography (VNG) system from the German company BioMed Jena. It is used to help assess disorders of the vestibular system that cause dizziness and vertigo, by recording and analyzing involuntary eye movements.

Nystagmus is an involuntary, rhythmic movement of the eyes. The vestibular system of the inner ear is connected to the eye muscles through the vestibulo-ocular reflex (VOR), so a problem in the vestibular system can make the eyes move on their own. A VNG test records these movements with video, and the clinician interprets them.

No. A VNG test is not painful and not invasive. The patient wears a video goggle and follows the clinician's instructions, such as looking at fixed or moving targets, and some tests involve head or body position changes. Caloric testing can cause brief, mild dizziness.

The eVNG USB records nystagmus during tests such as caloric and positioning tests. The eHIT USB is for the video head impulse test (vHIT), which analyzes the six semicircular canals. The two can be combined in one system on request.

Whether a VNG test suits a child depends on the child's age, ability to cooperate, and the clinician's judgment. BioMed Jena's eVNG datasheet does not describe a pediatric protocol, so ask iHear and the manufacturer if you need a pediatric application.

No. BioMed Jena's datasheet states that no other computer hardware is required. The eVNG connects to a PC or notebook through USB 2.0, and the software runs on Windows 10/11. BioMed Jena's website also lists a computer with a Full HD (1920 x 1080) screen and free USB 2.0 ports.

BioMed Jena's eVNG datasheet lists Windows 10/11. The eHIT USB requirements list Windows 10. Ask iHear to confirm the requirements for the software version you will receive.

According to BioMed Jena's datasheet, the eVNG USB is a USB-powered system with a binocular sample rate of 100 Hz, a resolution of less than 0.2°, and a 1280 x 1024 pixel camera. It uses infrared light (DIN EN 60825-1) and connects through Mini-USB type B to USB 2.0 (and one connection to USB 1.1). Its software runs on Windows 10/11, the camera and foot-switch cables are 3 meters each, and no other computer hardware is required. The datasheet lists the applied standards EN 60601-1 / 1-1 / 1-2 and the Medical Device Directive (MDD).

iHear (ihear.ae) is the exclusive authorized distributor of BioMed Jena devices, including the eVNG USB, in the UAE, headquartered in Al Ain with a branch in Abu Dhabi. To request a quote or a consultation, contact iHear on WhatsApp at +971 50 208 4569.

The price depends on the configuration, for example whether you add the eHIT USB, the KALORIstar Arctic, the VESTAR 100 USB rotary chair, or the optional extensions. Contact iHear for a quote based on your facility's needs.

Yes. According to the datasheet, an extension for optokinetic tests and rotational testing is possible at any time. The system is ready for optokinetic, smooth pursuit, saccadic, and antisaccadic tests, and can be connected to the VESTAR 100 USB rotary chair. The KALORIstar Arctic air caloric irrigator can also be added.

Both are used to record eye movements and assess the vestibular system. ENG (electronystagmography) records eye movements with electrodes placed on the skin around the eyes, while VNG (such as the eVNG) records them with infrared video cameras inside a goggle. Each method has advantages, and the choice depends on the test protocol and the clinician's judgment.

The basic version of the eVNG includes position and positioning tests, which are used in assessing benign paroxysmal positional vertigo (BPPV), one of the most common causes of vertigo. The clinician interprets the results and decides on the diagnosis.

Yes. BioMed Jena's datasheet states that the eVNG's improved algorithm is able to extract the pupil from the video stream even if the patient wears make-up.

Yes. The eVNG can be connected to the KALORIstar Arctic, an air caloric irrigator with active cooling, and the eVNG software can remote control the device, so the test can be run with one button.

It depends on the tests performed, but a basic test usually takes about 30 to 60 minutes. Patients are generally advised not to drive right after the test, especially after caloric testing, which can cause brief dizziness.

Not necessarily. BioMed Jena developed the optional manual pendulum test (MPT) to avoid the cost of an electrically driven rotary chair. It uses an ordinary office chair and the precise position sensors in the eVNG mask, and is used to check compensation after an acute loss of vestibular function.

Yes. On request, the eHIT and eVNG can be combined in one system, so one setup can run both VNG and vHIT tests.

A patient may be referred for a VNG test for recurrent or sudden dizziness or vertigo, suspected BPPV, symptoms that may suggest Ménière's disease (vertigo with tinnitus and hearing loss), suspected vestibular neuritis or labyrinthitis, or a balance assessment before or after inner-ear surgery. The referring doctor decides. See also our vestibular diagnostic devices page.

According to BioMed Jena's datasheet, the eVNG has a resolution of less than 0.2° and records both eyes at 100 Hz (100 frames per second). The nystagmus detection parameters can be changed to find even very small eye movements.

The basic version includes tests for spontaneous nystagmus, the caloric test, and position and positioning tests (such as the Dix-Hallpike maneuver used in assessing BPPV). It can be extended later with optokinetic tests and rotational testing.

BioMed Jena lists the eVNG as network ready: its application is based on a patient database that can be accessed over the local network. BioMed Jena's website also lists a GDT port. Ask iHear how to connect it to your hospital or clinic system.

Yes. BioMed Jena's catalogue lists the eVNG USB among the devices compatible with the eDM (Diagnostic Manager), so its data can be managed in one patient database together with other BioMed Jena devices.

A specialist ENT doctor, a neurologist, or an audiologist trained in reading vestibular reports interprets the results, not only the technician who performs the test.

BioMed Jena's eVNG datasheet lists the applied standards EN 60601-1 / 1-1 / 1-2 and the Medical Device Directive (MDD). CE marking is a European conformity marking, and use in the UAE is subject to the requirements of the relevant UAE health authority, so ask iHear for the current registration status and the documents for your facility.

iHear provides training for medical and technical staff on the eVNG's test protocols and software, plus ongoing technical support locally in the UAE. Ask iHear for the warranty and service terms that apply to your purchase.
